The conditions in which we all live, work, and play can have an impact our health. Omaha Better Birth Project can help connect you with community resources to help you and your family thrive.
Eligibility:
• Any family with children 5 years of age or younger are eligible
• There are no income or citizenship requirements
Resources:
Examples of community resources for families can include:
• SNAP or Medicaid Enrollment
• Food Pantries
• Rent/Mortgage Payment Assistance
• Utilities Assistance
• Clothing/Household Goods
• Mental Health Support
• Employment
How to Apply:
Click the link below to complete the screening and request social care referrals. After you submit the screening, our team will send your referrals through the Unite Us network. Make sure to complete the Unite Us consent at the end of the screening. Next, you will receive a phone call or email from each community referral requested. At this point, the community organization(s) will help you receive the appropriate assistance.
